Cheshire's Family Medicine teams provide comprehensive primary care to people of all ages, throughout their lives. We're located at:

So you can get the care you need, when and where you need it, we offer many ways for you to connect with your team. From same-day visits and a dedicated nurse line, to Virtual Visits and shared medical records with our emergency department, discover the ways we are ready to care for you.
Attention Winchester patients
Lisa Moulton, APRN, is leaving the Winchester practice of Dartmouth Hitchcock Clinics Cheshire (DHCC) on November 6, 2026.
Ongoing challenges, similar to those experienced throughout the country, have made it increasingly difficult to recruit and retain staff at the Winchester location, which is essential to maintaining the level of access, stability, and continuity of care our Dartmouth Health patients deserve.
Because Lisa Moulton is currently the sole provider at this location, we have had to make the very difficult decision to permanently close the practice location.
To make this transition as seamless as possible for you, we will identify a new primary care provider for your consideration prior to the clinic's closure. You will receive two mailed communications from us:
- The first letter was mailed on Wednesday, October 14. Read the letter mailed to patients (PDF).
- The second communication from us will be sent within 21 days with the name and contact information of the new primary care provider
