Contact us with myDH patient portal
If you are a returning patient, you may contact your provider securely through the myDH patient portal and the MyChart app. Use myDH to request an appointment or communicate with your health care team, and more. As a returning patient, you can also call our department's appointment phone numbers.
Nurse Clinic/Team F
Your provider may request you visit the Nurse Clinic at Cheshire Medical Center’s Team F for a special procedure or administration of select medicines, including services for anticoagulation. The highly-specialized nurses in this unit support all primary care teams. Apart from the Anticoagulation Clinic, you cannot make an appointment with the Nurse Clinic directly.

Registration for family medicine teams: you can register for all family medicine appointments on floor 1A, at the registration desk located between Teams A, B, and C.
Family medicine appointment hours: Monday through Friday, 8:30 am to 5:00 pm
