New patients
Thank you for choosing Cheshire Medical Center for your surgery or procedure. If you are a new patient, please ask your primary care provider (PCP) or other physician to forward a referral* with all pertinent medical records. If you do not have a PCP or referring doctor, please call our appointment line to talk with our team. Please see our referrals and precertifications page for more information about referrals.
Once we receive your referral, we contact you to schedule your appointment. The scheduling secretary will explain your appointments to you in detail.
*Some procedures, such as bariatric surgery or some vascular surgeries do not need a referral, depending on the requirements of your insurance company. Please call those offices directly for more information.
Use myD-H to contact your providers
If you are a returning patient, you may contact your provider securely through the myDH patient portal and the MyChart app. Use myDH to request an appointment or communicate with your health care team, and more.
