Appointments and Referrals

New patients

Thank you for choosing our care team for your medical care. If you are a new patient to Cheshire Medical Center, please ask your primary care provider (PCP) or other physician to forward all pertinent medical records. If you do not have a PCP or referring doctor, please call our appointment line to talk with our team. Please see our referrals and precertifications page for more information about referrals, as some insurance companies need referrals for some services we provide.

If a referral is needed, we will contact you to schedule your appointment once we receive it. We may recommend testing before your appointment, which we can help you coordinate. The scheduling secretary will explain your appointments to you in detail.

Existing patients

If you are a returning patient, you may contact your provider through the myDH patient portal and the MyChart app. Use myDH to request an appointment or communicate with your health care team, and more. As a returning patient, you can also call our department's appointment phone numbers.

All other scheduled appointments are accessed through the Women’s Health entrance off our central staircase and elevators.
