Appointments and Referrals

New patients

Thank you for choosing our care team for your medical care. If you are a new patient, please ask your primary care provider (PCP) or other provider to forward a referral with all pertinent medical records. If you do not have a PCP or referring doctor, please call our appointment line to talk with our team. Please see our referrals and precertifications page for more information about referrals.

Once we receive your referral, we will contact you to schedule your appointment. We may recommend testing before your appointment, which we can help you coordinate. The scheduling secretary will explain your appointments to you in detail.

What to bring to your appointment

If this is your first visit Cheshire’s neurology team, please bring the following information with you:  

  • Medications and/or a medication list
  • Previous neurology records and related test results, if you are not a patient from Cheshire or another Dartmouth-Hitchcock Health location.

Returning patients

If you are a returning patient, you may contact your provider securely through the myD-H patient portal and the MyChart app. Use myD-H to request an appointment or communicate with your health care team, and more. As a returning patient, you can also call our department's appointment phone numbers.
